Pregnant women who do not receive adequate epidural analgesia i.e. pain control are at risk for difficult deliveries. Epidural anaesthesia involves infusing medications through a thin catheter inserted into the space that surrounds the spinal cord in the lower back, which causes loss of sensation in the lower half of the body. To assess whether or not sub-optimal pain control before delivery could lead delivery problems like the need of caesarean section , researchers from Canada examined data from 1,756 women in labour who had taken part in a trial involving epidural analgesia. Sub-optimal pain control was seen in 191 women. Poor analgesia was found to increase the risk of caesarean section by nearly threefold and the risk of tears around the vagina by almost twofold. The results also showed that it was impossible to improve delivery outcomes by attempting to catch-up by adding analgesia. If the initial analgesia was sub-optimal, then the woman was at risk for a difficult delivery. Although the exact mechanism underlying this finding is uncertain, the researchers suggest that failure to achieve or sustain adequate pain relief with an epidural analgesia might signal an increased risk of caesarean or instrumental delivery.
