Anxiety and post-surgery delirium

Older patients often suffer delirium after heart surgery, but this seems to be unrelated to pre-surgery anxiety and depression. Delirium is a common psychiatric complication after cardiac surgery. It results in decline in attention-focus, perception, and cognition. Special attention must be paid to delirium after heart surgery, because the syndrome is associated with poor outcomes, including higher rates of post-surgery complications, longer hospital stay, and higher death rates. To investigate the prevalence of pre-operative anxiety and depression and their relationship with the occurrence of post-cardiac delirium, researchers from Belgium studied 104 elderly patients, aged around 70 years, admitted for heart surgery. Anxiety and depression were measured in all the participants before and after the surgery. It was found that 26 percent of the patients suffered with postoperative delirium for 2 days. Before surgery, 56 percent patients had symptoms consistent with anxiety and 25 percent had genralised anxiety. Overall, more than 15 percent of subjects reported pre-surgery depressive symptoms. The above findings indicate that despite the high prevalence of preoperative anxiety and depression, it is not related to the occurrence of delirium after surgery.
