There is treatment for infertility in men, and semen test is needed to know if a man is infertile. Masturbation has no bearing on fertility and is not a cause. There is no need for simultaneous orgasm - in fact orgasm is not required for conceiving. If you feel that semen flows out, it is not semen but the secretions which flow out. So first get his semen checked, and if it is normal, get yourself checked by a gynaecologist.
