What is the treatment for typhoid fever?

Q: One of my friends has tested positive for typhoid bacteria (S. typhi). Can she take the typhoid vaccine now after getting the disease along with antibiotics like Ciprofloxacin or Amoxycillin, what ever you say? Which tablet of Ciprofloxacin should be taken? She is 23 years old and has no allergy to any drug.

A:Blood culture showing S. typhi is definitive test for typhoid fever. The patient should be urgently placed on Ciprofloxacin (brand name Ciprobid) 500 mg twice daily for 7 days along with Paracetamol (Crocin) 500 mg 3 times daily to control fever. The fever will come down by the 5th day evening at the most. Amoxycillin is not the drug of choice. Earlier, Chloramphenicol was the drug of choice but most typhoid bacteria have become resistant to this drug. Please keep in mind that Ciprofloxacin is not to be used in pregnant women. There is no use of giving typhoid vaccine when the patient is suffering from typhoid fever. Typhoid vaccine contains attenuated S. typhi bacteria that will make the condition worse. In any case, typhoid vaccine is not to be used during any acute fever.
