Why do I have irregular periods?

Q: I have irregular periods, typically every 40 days since the past 3 months. My doctor suggested some blood tests to ensure if I am ovulating. Day 2 Estradiol test result was 150. Please tell me if this is normal? What treatment should I undergo, since I want to get pregnant soon.

A:Only one test result cannot be taken in isolation to decide what treatment should be prescribed. You need to check cause for anovulation (if present). Following that the appropriate treatment is prescribed. The best way to check whether or not you are ovulating is to undergo ultrasound monitoring of ovarian follicle and endometrial thickness. If ovulation is documented, you need to get a serum progesterone level done 3 days after ultrasound documentation of ovulation (for confirmation) and again 7 days later to check for adequacy of progesterone secretion by the corpus luteum.
