Has my wife had a miscarriage?

Q: My wife is six weeks pregnant. She was suffering from PCOS earlier. Her LMP was in the third week of September. A few days back, her TVS USG was done and the radiologist informed that the fetus is normal in shape and size, but he could not detect the heartbeat. He said it is a case of miscarriage although there has been no bleeding / no pain / no unusual symptom. Will you please let me know if the radiologist is right or is it too early to see the heartbeat? Are there any effects of USG (once or repeated) on the fetus?

A:By TVS one can usually see the heartbeat by the sixth week of pregnancy. However, there is no harm in waiting for one more week before repeating the scan to confirm. Symptoms of miscarriage are not always present. It is easier to calculate duration of pregnancy if ovulation monitoring was done during the cycle when she conceived, and exact date of ovulation known, as your wife has PCOS (and I guess had irregular menses). USG will have no harm on fetus - in any case it is indicated in your wife's case.
