Why does my shoulder makes a grinding/cracking noise?

Q: I am a little concerned about my right shoulder. I am a former tennis player and still play recreationally. I currently weight lift daily. The concern now is not the pain but the fact that my shoulder makes this horrible grinding/cracking noise when I roll it around. It seems that the cracking is more from my collar bone area. The only discomfort I have is at night. There is also some discomfort when I lift heavier weight in the gym. I am only 28 years old. I would like to know if this is something I should be treating and how to prevent future problems?
