How to manage glomerulonephritis?

Q: I have a creatinine level of 2.7 mg/dl currently. This has steadily risen from 1.48 about 6 months back. I have a protein loss of 1.8 gms per 24 hr (urine test). Haemoglobin is 9.2 g/dl. I also suffer from hypothyroidism; the TSH level was 16 i.u. when I started on Eltroxin about 3 weeks back. Is this nephrotic syndrome? Is there any medication that can help me or do I have to just wait and watch?

A:From the reports sent it is evident that you have a glomerular disease with kidney function impairment which is related to the glomerulonephritis. If the ultrasound abdomen reveals a normal kidney size then I would recommend a kidney biopsy if it has not been done already. The specific medicines, if any, can only be started after seeing the kidney biopsy. Kidney biopsy should include immunoflorescence examination also. The high blood pressure, if any, should be controlled affectively with ACE inhibitors preferably.
