How to titrate the eltroxin dose to reach normal TSH values?

Q: My mother, 66 years old, is having a TSH level 0.14. Last month the TSH level was 91. After taking eltroxin 100 mcg 1 tablet per day for six weeks it has reduced. I went for another TSH test, which showed 0.14 now. Please suggest what I should do now. Should the dose be reduced to titrate the range?

A:The target TSH values during treatment of hypothyroidism is mid-normal TSH. A simple rule is when TSH is low reduce the dose and if TSH is more increase the dose. At one time dose change should not be more than 25 per day. Next TSH should be done after 6 weeks only. Once TSH stabilises even once a year TSH is good enough. In this case reduce the dose to 75 per day. If this brand is not available then you can switch over to Thyronorm 75 per day.
