What are the reasons for anosmia (loss of smell)?

Q: My cousin is suffering from loss of smell. The doctor has said it is anosmia . We want further investigation and diagonise with certainty. My question is - where(hospital) and whom (the specialist doctor) should we approach for the said investigation and necessary treatment. Also what are the sucess rates for this desease. Kindly give your expert opinion at the earliest.
