Q: I am a 32 years old woman having a six and a half year old child. Last year I had done IUI pregnancy, which resulted in miscarriage. The cause detected was low protein S level (46). I have conceived again by IUI 4 months back. The doctor has put me on Clexane 40 uts daily dose. Now the first trimester is over and my protein S level is 55. In recent ultrasound my cervix is 2.9 cm, a bit on the lower side. Is Clexane safe throughout pregnancy? Can drugs increase it or stitch is the ultimate solution? Is there any side effect of Clexane on fetus and can surgical delivery be harmful for me with Clexane?

A:Clexane is safe in pregnancy and is required in your case with low protein S levels. This needs to be stopped at least 24 hrs before your delivery / caesarean. For your cervical length, you can start on progesterone (Injections or vaginal tablets) if you are not already taking them. A stitch is usually recommended if cervical length is < 2.5 cm.
