Why does my wife's foot swell often?

Q: My 53 years old wife has swelling in her left foot from the ankle to the calf. The affected area is inflamed and painful. She also has high fever. The swelling reduces at night but comes back as soon as she moves around. This lasts for about 4 to 5 days. When the problem first appeared about 20 years ago, treatment for filariasis was given. However, the condition has continued on a regular (once/twice a year) basis. She has also tried out homeopathy treatment. Can you please help?

A:Intermittent painful swelling from the ankle to the calf since 20 years associated with fever, the likely causes are as follows:

  1. Gout
  2. Monoarticular arthritis
  3. Thrombophlebitis
  4. Lymphangitis
I will suggest that you meet a local Physician or if available a Rheumatologist after getting the following investigations:
  • Serum Uric Acid
  • ESR, C - reactive protein (ultra sensitive)
  • Doppler study for both artery and venous flow affected leg
  • X-ray Lt Leg AP and Lat
