My 37 years old husband suffered from tuberculosis four years back and after a few weeks of medication the doctor asked for a HIV test. The test report was positive. So, he started taking medicine for HIV and took it regularly. After a month we tested his CD4 count, which was same (low). But suddenly he stopped taking the tablets for HIV (for 2-3 months) and once again he got done CD4 count, but this time the count was high. Now for the last 4 years he has not been taking any medication and he is in good health. I am worried for him. He is not ready to accept that he has HIV because of the variation in CD4 count. Please advise. ]]></description>

 <title><![CDATA[Why do I feel drained out after being treated for Hepatitis E? ]]></title>

I am a 28 years old male. When I returned from India this summer, I developed jaundice and on diagnosis, it was revealed that I had been infected with Hepatitis E. My ALT and AST levels were quiet high. It took three weeks until my blood tests were normal and the ultrasound reports were also normal. My doctors advised me to lead a normal life, which I followed. But now even after a month and a half after the infection, I feel tired and drained out at times. Also I am a marathon runner and maintain good level of fitness with regular muscle and cardio training. Even now I go for running (10-15 km) but then I feel weak and require time to get back my energy levels. Along with this I also feel bloated. My doctor says it's normal to experience these symptoms, which will go away on their own, and there is no cause of worry. Could you please advise? ]]></description>

 <title><![CDATA[Is it necessary to vaccinate the child within 6 months of age? ]]></title>

My 5 months old son has been given the first dose of DPT, HBV, OPV and HiB vaccines when he was one and a half months old and the second dose at two and a half months of age. Now the third dose was supposed to be given at three and a half months, but we moved to Dubai and here the doctors give these three vaccinations at two months interval rather than one month as in India. So they gave pneumococcal conjugate vaccine (PCV) when we went for the third dose of the vaccination. They said that the third dose of DPT, HBV, OPV, HiB would be given two more months after PCV that will be three months from the second dose. Is this interval of 4 months between the second and third dose fine? If not, how soon after the PCV can we give the third dose? Is it necessary to give all the three doses of DPT, HBV, OPV and HiB vaccine within the first six months only? Please advise. ]]></description>

 <title><![CDATA[How can seizures be managed in children? ]]></title>
 <description><![CDATA[
My 9 years old son is suffering from partial seizures for the last one and a half years. Different medicine combinations have been tried on him viz Clobazam, Oxy-Cabazepine, Lamotrigine, sodium valproate (Chrono), etc. Now he has been put on sodium valproate (non-chrono) with Clobazam. There is considerable relief as far as severity and frequency of seizures is concerned. He is a hyperactive child and experiences at least one seizure daily immediately after returning from school because of anxiety and excitement to share school happenings with us. Also, he is much interested in toys, cartoons on TV and computer games instead of studies and outdoor games. His bowels are erratic and he spends a long time in the toilet and day-dreaming. He does not like studying and the very mention of studies makes him angry and scared at times. Even though he is an intelligent child and well aware of all happenings for a kid of his age, he does not take notes in the class, nor completes his class-work. As a result, bulk of the studies including homework and explanation part has to be done at home that further restricts him from enjoying creative and leisure activities at home. This limitation imposes a lot of stress on him and has acted as a trigger for seizure at times. He loves us and tries to please us but failure to do so induces a sort of fear in him triggering the seizures. No amount of persuasion, cajoling, assurances and even interactions with clinical psychologist has helped the matter. Please advise. ]]></description>

 <title><![CDATA[Why do I need to use reading glasses? ]]></title>

I am 38 years old man having reading glasses with a power of 1.75 in both the eyes, which I checked two weeks ago. I started wearing reading glasses 2 years ago with a power of 1. The problem started 3 years ago, after working for 5-6 hours I feel blurring of vision and if I close my eyes for 5 minutes then again it is fine for next 10-15 minutes but this problem starts again. The doctor gave me some homeopathy pills, which I took almost for a year but since there was no relief the doctor gave me glasses, as I was not comfortable doing any work. Now again the same problem has started so I got my eyes re-checked and have been given reading glasses with a power of 1.75. The doctor told me that people usually use reading glasses only at 40 years of age or above but due to my problem I need the reading glasses. Would increasing number be a problem at a later stage? Is there any treatment or surgery by, which this problem can be sorted out? If I donate my eyes, will the person receiving them also have same problem? Please advise. ]]></description>

 <title><![CDATA[How can recurrent fever be cured? ]]></title>
 <description><![CDATA[
My 29 years old husband is suffering from recurrent fever for the last 3-4 months. It started with high fever with chills, so he was given anti-malarial drugs and he felt normal. Then again after 15 days he got the fever with chills then he was given antibiotics. Again after 15 days of the treatment, the same thing happened and he was given anti-malarial Flacigo. The 4th time he was admitted to the hospital for 5 days and was on anti-malarial medication. When he got  fever for the 5th time he was given Lariago DS, Falcinil and Doxy then after that Primaquine for 5 days twice daily. His test result showed ESR-35, spleen-mildly enlarged, liver infection (when admitted to hospital 4th time) but the malaria, tuberculosis (TB) and dengue test were negative every time. His red blood cells and white blood cells were low, platelet count was 2.4, but when admitted to the hospital 4th time the count was 2.6 then it became 1.2 and it recovered up to 1.57, mantoux test-negative, blood culture and urine culture – normal. During fever he suffers from body ache, joint pains at times and leg pain whenever he exerts. In a nutshell, he gets high-grade fever with chills and after taking medicines he becomes fine but after 2 weeks of treatment he again gets fever. What should we do now? How can it be cured? Please advise. ]]></description>

My 7.5 years old son is an active and smart child. He is doing well in his studies. During his recent annual vacation he visited India and spent around 45 days at his grandparents house where he enjoyed a leisure life by getting up late, watching TV mostly, eating all types of food and slept a lot. When he came back home, we noticed a sudden change in his attitude namely he is always sleepy in the daytime in front of TV/ in car / in school wherever he goes. On the other side, we have also noticed that he has night terrors / sleep terrors at nights. He goes to bed at 9 pm (which is unusual compared to past, which was normally around 11 pm) and go to deep sleep until 1 am and then he started moving his body by rolling and blabbering unrecognised words and suddenly wakes up and tell us that somebody is standing near the door and some insect in bed etc. We immediately tell him to sleep then he immediately falls in sleep. Again after 30 minutes this may continue and finally he goes to sleep early morning around 3 am and sleeps till 6.30 am then we wake him for school. When we ask him what has happened and why he talked then he could not recollect what he said last night. During daytime we have noticed some slurred speech while talking. This is happening for the last 2 months after he came back from his grandparents home. Previously he was active / smart / too much talkative boy. Please help. ]]></description>

My one-month old daughter is having hypothyroidism since birth. She had high bilirubin count (19.9 mg/dL) at the time of the birth, due to which she had infant jaundice. The doctor suspected thyroid problem for high bilirubin and hence we did a TSH test. Six days after birth, her T4 - 1.97 ng/dL, T3 - 4.99 pg/mL, TSH - 13.76 uIU/mL. After 17 days we started giving her Thyroxine 0.025 mg daily. Then we tested her thyroid levels again after 2 weeks in which, T4 was 1.61 ng/dL, T3 - 4.58 pg/mL, TSH - 2.62 uIU/mL. So, we gave her Thyroxine on alternate days for 4 days. Now last week her thyroid levels were - T4 - 0.94 ng/dL, T3 - 4.85 pg/mL, TSH - 9.76 uIU/mL. Is my child still having hypothyroidism symptoms? Do I need to continue Thyroxine? Are there any side effects of these tablets? What is the normal TSH value for infants? ]]></description>

My 35 years old brother-in-law has alcoholism, though not on a regular basis. He got married 9 years back to my sister and started drinking alcohol heavily after the second year of marriage. After drinking he starts abusing his wife and all relatives in filthy language and making false and rubbish statements/ accusations. This bout lasts for about one week to ten days. After that he becomes normal and leaves drinking. Such episodes keep on coming in every 4 to 10 months (infrequently). During such bouts he recalls all the sundry incidents in the past and keeps reciting them telling whoever is present. This way he tries to justify all his abnormal behaviour. After the passage of the bout, he apologizes to all of us and requests us to unify with his wife and children. For the last 9 years this has happened at least 25 times. We had taken him to psychiatrist for treatment of alcoholism. Assuming that he is a normal person doing such things deliberately, we took the case with police and women's help groups many times. But there has been no improvement. His strange behaviour still continues. This is causing a lot of hardship to his family and us (as close relatives). Please suggest. ]]></description>

My 31 years old wife had primary infertility and improper periods. Her prolactin level (PRL) was 210 ng/ml. Her MRI indicated 1.5 x 2 cm cyst and was operated for trans-sphenoidal pituitary macroadenoma successfully five years back. She gave birth to a baby the next year. Then again her periods displaced and PRL was found 57.1 ng/ml. MRI was done and 1.1 x 0.7 cm sized peripherally enhanced cyst in the right half of the sella and thin enhancing soft tissue evolving the ICA in its left half (recurrent lesion) was found. So, she started taking Cabergoline 0.5mg 1 tablet per week. Now the levels are below 11 ng/ml since then. For how long she would be talking this medicine? Because the moment she stops taking medicine her PRL increases. Is there any side effect of the medicine if she continues taking it for life long? What else we can do to reduce PRL? Will size of the tumour remain same if medicine is taken? What could be the implication of this when menopause occurs? Can we go for second baby? ]]></description>

Often we have heard her saying that we love the other child more than her, which is not the case. It is getting difficult with each passing day. She is always crying and we don't know what the reason is. In fact now complaints from my mother-in-law and sister-in-law are increasing every day. In marked contrast, she is a very quiet and well behaved child in school and in fact, the remarks on her report card read that she is a very well-behaved and obedient child and very good in her studies. She has got ‘A' grade in almost all her work. Please guide as to what steps need to be taken by both of us and where are we going wrong. We feel very bad after slapping her and now don't even want to have a second child. ]]></description>

 <title><![CDATA[How can my daughter's kidney problem be treated? ]]></title>

My 7 years old daughter is having recurring urinary tract infection (UTI) for the last 3.5 years and is treated with crocin and appropriate antibiotic medicine as and when the fever comes. Recently, MCUG test was done and reflux of grade III was found on the left side. DMSA tests suggested that 22% left and 78% right kidney is functioning and also there are lot of scar marks on the left kidney. However, right kidney is reported as normal. Two months back the doctor advised prophylactic medicine for the next 2 years and he asked us to wait as he says such refluxes gets automatically ok as the child grows. We followed his advice and immediately put her on Septran 480 mg daily. In between these 2 months, we sent the same urine sample for testing to 2 different labs for urine routine and culture. Report from lab 1 - WBC - 8-10, epith cells 3-4 and growth of E. coli in excess of 1 million. Report from lab 2 - WBC - 5-7, epith cells ++ (in fair number) and growth of E. coli around 10,000. She is also getting mild fever in between. What should we do now? As recurring UTI and non-sterile urine reflux may be causing further damage to the kidney. I have read somewhere that endoscopic and open surgery options are available nowadays. What is the success rate? Which hospital in Delhi provides this treatment? Please advise. ]]></description>

I am a 41 years old man who has undergone endoscopic surgery for the removal of nasal polyps once and another similar surgery for the correction of deviated nasal septum (DNS) around five years back. Now, I have developed the symptoms of nasal polyps again as the polyps are growing and I am finding it very difficult to breath. Most of the times I am taking breath through my mouth. Can I go for endoscopic surgery for the removal of polyps again? Is there any side effect or any problem if I go for the surgery again? What are the chances of its recurrence? ]]></description>

He is not interested in replying to the questions that we ask and he mostly repeats whatever is told to him. He, however, uses proper sentences when he has to convey his requirements. He knows the
advertisements shown on the TV or the movie trailer that is running on the TV. He correctly answers things that his mother taught him while preparing him for his Nursery admissions. He is fully aware of different fruits, vegetables, shapes, colours and other things that a child of his age should know. We are Bengalis and were speaking in Bengali at home till now. We had admitted him to a Play School a year back (after which he spoke his first word). How do we encourage him to start speaking like any other child of his age? ]]></description>

 <title><![CDATA[Why does my son faint while crying? ]]></title>

My 9 months old son tends to cry vigorously sometimes and if not attended to,  he faints. This has happened three times (1st time in the 4th month, 2nd&amp;3rd time in the 9th month). When he faints his eye balls go up; and the face and lips colour change too. He is an active and healthy child otherwise.
