Q. My CT scan (CECT) of the chest reveals evidence of a large cystic SOL in the left lobe region with an internal fluid level suggesting possibility of an infected brochogenic cyst. Differential diagnosis includes loculated hydropneumothorax. What are the treatment options?

A.  The symptoms and past history and x-rays often clearly favour the possibility of bronchogenic cyst vs Hydropneumothorax. CT should further clarify the issue. Treatment is surgical removal in the former and ICT drainage in the latter.
