Should I take these drugs for cerebellar atrophy?

Q: The MRI of my brain shows that I am suffering from cerebellar atrophy. All blood reports are normal except HLA B27 - Indeterminate. I have vertigo for the past 4 years, pain at the back of my head, decreased memory and lack of concentration. There is pain in joints and muscles (especially finger joints, wrist, spine and knees. I also have a peculiar sensation in legs, hands and lower portion of the body. I am not able to follow other's talk properly and am not able to express myself properly. I also suffer from depression, have a vomiting tendency and unpleasant sensation on the head, hand and body. My doctor has prescribed me Amixide H - 1 tab daily; Betacap TR (40); Feliz S (5); Vertin (16) SOS. What are the functions of these medicines in the human body? What are the side effects of these? Let me know whether Clonazepam appears in the internationally approved data bank and what is its action on the human body including side effects?

A:1. At the age of just under 50, to suffer from primary cerebellar atrophy is quite rare. Since the diagnosis is based only on one MRI, you may wish to get this test repeated from another radiologist to avoid bias. Unlike x-ray, MRI is a real time test. Hence the chances of observational error can not be ruled out. 2. HLA B27 stands for Human Leucocyte Antigen. This test was done to see if you are suffering from Rheumatic disease in view of pain in joints etc. In your case the test is negative which is good. 3. Amixide-H is a Fixed-Dose Combination of two medicines: amitriptyline (an antidepressant) and chlordiazepoxide (an anxiolytic agent). Please see general statement on FDCs at the bottom of this response. 4. Betacap TR is a brand name; it contains propranolol. It belongs to a group of medicines called beta blockers. After the launch of better selective beta blockers (such as atenolol sold under the brand name of Aten), non-selective propranolol use is not in vogue. I do not know the reason for prescribing this medicine to you. It is used in high blood pressure, angina, migraine and sometimes for tremors or anxiety. 5. Feliz S is the brand name of a medicine called escitalopram, a very potent medicine used in depression. Feliz S does not appear on the databank of quality products. The only brand that appears on international databank of quality products is Cipralex by Lundbeck - the innovator of the medicine. 6. Vertin is quite appropriate in your case. 7. Clonazepam brand that appear on the international databank is Epitril by Novartis. Zapiz does not appear on the list. It is normally used for epilepsy though some psychiatrists also use it for depression. Instead of taking so many medicines with so many side effects at this age, it is suggested that you try Yoga and Pranayam. Fixed-Dose Combinations (FDCs): Medicines are discovered individually and are supposed to be taken separately. A huge number of irrational, illegal combinations of drugs are being sold in India; quite a few without mandatory approval of the Drugs Controller General, India (DCGI). Except in a few cases (such as TB medicines), it is always better to take medicines separately so that dosage can be adjusted and side effects monitored.
