Why am I unable to conceive?

Q: I am a 24 years old married female. I have not conceived yet but have been trying for a baby for the last one year. I consulted a doctor, who told me that I have no gynaecological problem. My husband has also undergone sperm analysis, which reports that his sperms are 60% active. My menstrual cycle is also regular. Why am I unable to conceive then?

A:It is difficult to comment without examining you personally. You need to have follicular monitoring by ultrasound and timed intercourse with a postcoital test done. In the Postcoital test, you are advised to have intercourse at the time when the follicle is at a good size (ready for ovulation) and you should report to the gynaecologist within 4 hours (maximum 6 hours) after sex. The gynaecologist will do a speculum exam, check the quality of your cervical mucus , take some and spread it on a slide and examine under a microscope to check how many sperms are there and their motility and look for pus cells and any infection. Depending upon these results you can be advised whether or not you require IUI. Of course, before IUI you need to have an endometrial biopsy done, followed by tube testing (to check for potency) with a special X-Ray (HSG). Kindly meet an experienced infertility specialist.
