Are Penicillin injections necessary to prevent RHD?

Q. My younger brother is taking Penidure LA 12 penicillin injection that is given for prevention of rheumatic heart disease. Now it is not available in the market. What is the substitute he can take? He is taking the injection from the age of 6 and now he is 19 years old and his doctor has advised him to take the injection till the age of 21 years. But since he doesn't have any problem (joint pains) now, should he continue this or consult a doctor for a complete checkup?

A.  Penidure LA 12 is the brand name; the name of the medicine is injectable Benzathine penicillin G 1.2 million units. This medicine is also sold under the brand name of Pencom 1.2 million units by Alembic. There is absolutely no difference between Penidure LA and Pencom. Your doctor is right. The medicine should be taken at least up to 21 years, may be longer, even if there are no symptoms. Not taking the medicine can result in infection, which can damage the heart valves due to rheumatic heart disease (RHD).
