Why does my husband’s semen remain stuck on his penis?

Q: I am a 35 years old woman trying for conception for over a year now. My husband’s sperm count is a little low but my ovulation occurs regularly. We have noticed that his sperms are stuck to the penis (in the same spot on the top of the penis) immediately after intercourse. Is it possible that semen is not entering the vagina and staying on his penis only? I have also noticed leakage of semen for a couple of hours after having intercourse. Can this be the cause of having trouble in conceiving?

A:It is normal to have semen at the tip of the penis after intercourse. The normal concentration of sperm is about 15-20 million per cc semen. As long as your husband is able to ejaculate into your vagina and his semen volume and counts are normal there should be no concern regarding sperm entering or reaching its appropriate destination. Few drops of semen seen few hours after intercourse is also normal and should not be a cause for concern. You and your husband should be evaluated for infertility by a urologist and a gynecologist if it has been more than a year of every other day intercourse around your time of ovulation.
