Q: My 25 years old sister has been suffering from high fever (101 - 102 degree Fahrenheit) for the last one week. The fever is not persistent and generally occurs at night. She is also suffering from dry cough and headache. There is no throat pain, no burning sensation while urinating, no body ache, no diarrhoea and no vomiting. All pathology tests done have came out normal. The doctor has prescribed OratilCV 250 BD, Cifran 500 BD, Dolamide BD for seven days and T Rezatnin Forte BD for three days. Two months back, she had the same symptoms with abdominal problem for which she was admitted to the hospital for three days and was given antibiotic treatment (mikacin, sefup, accigrip, pantocid). Why is she getting recurrent episodes of fever?

A:One possibility is that she had an infection like typhoid in February, which was partially treated and has now relapsed. Sometimes, the culture and other reports may not give an answer if the patient is already on antibiotics. Apart from the tests that have been done, please repeat a malaria antigen, ultrasound upper abdomen, chest x-ray-pa, stool routine and culture, widal. Continue on the current set of medication and maintain a temperature chart. If the fever resolves, then we don't need any further investigations. In case fever persists, more sophisticated tests for chronic infections would need to be done.
