Why does my shoulder makes a grinding/cracking noise?

Q. I am a little concerned about my right shoulder. I am a former tennis player and still play recreationally. I currently weight lift daily. The concern now is not the pain but the fact that my shoulder makes this horrible grinding/cracking noise when I roll it around. It seems that the cracking is more from my collar bone area. The only discomfort I have is at night. There is also some discomfort when I lift heavier weight in the gym. I am only 28 years old. I would like to know if this is something I should be treating and how to prevent future problems?

A.  Sounds like the patient is suffering from arthritis of the acromioclavicular joint. In such cases, the joint is arthritic and is painful. Such conditions are common in over head sports athletes and weight lifters. Treatment is a local injection into the joint or in recalcitrant cases, keyhole surgery.
